The card is presented whenever you visit a Basic Health Unit, hospital, or other health service. However, it is common for people to lose this card, so it is important to know how to request your second copy.

The SUS Card
The SUS card is the form of identification that the holder is registered in the health program. It was developed by the Ministry of Health and in recent years has helped thousands of people gain access to quality and specialized medical resources.
The card is usually issued at a local health center. For this, you must bring a photo ID as well as proof of your residential address. This way, the service becomes increasingly accessible.
How to request the second copy
The second copy can be requested online through the Portão Cidadão (Citizen Portal). To use this tool, you need to already have a SUS Registration. If you don’t have the registration, simply access the portal and register.
If you already have it, you just need to search for your name. The system will then filter and confirm your presence in the records. After the card is issued, you can choose to print it or use it online.
If you want to use a printed version, you will download the file in PDF format and print it as soon as possible. If you prefer to request it in person, just go to a local health center and follow the same process described above.

How does Conecte SUS Cidadão work?
Conecte SUS serves several purposes, such as checking medical test results, scheduling appointments, getting information about all the services offered by SUS, and, of course, generating your virtual SUS card to present during your appointment.
Currently, the app is also functioning as an information source for groups eligible to receive the COVID-19 vaccine, but registration is not mandatory to access this information.
More important than knowing what the Conecte SUS app is, is knowing how you can register and keep using it. We will cover more about this in the next section.
How to register on Conecte SUS?
To register on Conecte SUS, you need to follow some steps described below.
- First, access the Federal Government website to create your registration. You will be asked to provide your personal information, such as full name, CPF (tax ID), and date of birth;
- After following the step-by-step instructions on the website, your account will be activated, and you will use it to log in to the app;
- Finally, download the app from the App Store or Play Store and log in using your CPF and the password you registered on the website.
From there, you will be able to schedule exams and appointments, check reports of exams you have already done, and even track the progress of vaccination in your region.
On the website, you also have the option “Service Types,” which basically works like a GPS for your area, locating hospitals, care centers, pharmacies, and health units, making access to these places much easier.
